Grad Nite Essential Forms

• Combo: Ticket & Agreement Form 2012 - Conduct Agreement is required to be able to attend Grad Nite. This form must be read and signed by both the participating student and their parents/guardians. The Ticket Form is to be completed and submitted with payment in order to attend Grad Nite.




Drop off the Combo Form with signed agreement and payment, in the Grad Nite box in the MVHS Office, or mail to: MVHS Grad Nite, 25108 Marguerite Parkway #105, Mission Viejo, Ca. 92692.

• Medical Form 2011 - It covers the Parent / Guardian / Physician Request for Medication. Anyone who takes any medicines for any medical reason ( diabities, epilipsy, chemo, radiation, migranes, etc...) needs to fill this out and take it to the office to be dropped into the big blue box on the office counter.
• Volunteer Wish List 2011 -To volunteer to work on a committee, to be a Chaperone, or even donate items for prizes!!
• Business Community Solicitation 2011 -We invite your participation by asking for your donations -- monetary contributions, raffle gifts, door prizes, silent auction items, gift cards and/or store coupons.
